Principles of Antihypertensive Drug Therapy Do not delay drug treatment. Lifestyle changes often do not succeed. Follow the patient at 6-week intervals until control is achieved. A six-month period of undertreated hypertension increases cardiovascular morbidity.1 1Julius et al. Lancet 363:2022-2031, 2004

Summary: Treatment goals according to the 2017 guidelines: Adults 130/80 In all patients, initial therapy should include: ACEI, ARBs, (dihydropyridine) CCB and/or thiazide diuretics Black patients: Start with thiazide or CCB Patients with CKD: Initial therapy should include ACEI or ARB Use combination therapy if blood pressure is >20/10 over goal Do not combine ACEI and ARB
