What is the Forms & Surveys Module? A web-based tool that makes it easy to create forms & surveys through the web. Collect & share results.

How it works Database Email Advanced Reporting Data Export The user browses to the organizations site and fills out the form or survey online. The user submits the form or survey to the Schoolwires server through the web. The form or survey results are processed The Schoolwires server automatically appends the submission to the Schoolwires database. The Schoolwires server sends email to the form or surveys owners notifying them of the new submission. (OPTIONAL) The Schoolwires server allows form or surveys owners to generate numerous reports. The Schoolwires server allows form or survey owners to export the data to a CSV format. The format will work with virtually all database formats including Excel, Access and FileMaker Pro. Simple Form Creation 1) Creating a form with Schoolwires is a snap. Simply click the New Form button to create your new form or survey. 2) Enter your form's title and click the Save button. 3) After your form is added, start building your form from a library of over 20 item types: matrix, multiple choice, open-ended text boxes, checkboxes, HTML, images and more. 4) In this example, we chose to add an Open Ended - One Line question. Simply type your question and select options then click Save. 5) As you can see, your item has been added to the form. If you wish to add more items to your form, click the Insert button. If you wish to view your form, click the View Form button. 6) Your form or survey can be published in your Schoolwires website layout or in a new window.

Advanced Reporting Powerful reporting including summary and individual reports Easily share results with others Take advantage of our advanced reporting, which includes summary reports, individual results, lists of respondents, report filtering, and data export. Easily share your survey results with others. Each report share allows for granular control over which reports are shared, and what functionality is available for that user.

Advanced Security Show or hide forms or surveys by the user’s role Restrict user to only one response or multiple responses Restrict access to forms or surveys by IP address Rest assured that your surveys are secure. Each survey is assigned one of 7 respondent access levels to ensure the appropriate security for that particular survey.

Page Conditions & Answer Piping Guide users through a multiple page form based on their answers. Use powerful page condition logic to optionally display any page in a survey based on responses to one or more questions the respondent has already answered.

Data Export Export data to CSV format Export data into multiple schemes Export data from any survey into CSV/Excel in one of three formats. Includes dozens of options for exporting additional data. The CSV format will work with virtually all database systems including Excel, Access and FileMaker Pro.
