2 2 Objectives Create and modify an online form template using form controls. Insert online form fields for text boxes, check boxes, and drop-down lists. Insert a table onto a form. Format responses in text form fields.

4 4 Terms Used in This Lesson Elements – Part of XML code that describes the information contained within the XML document. Extensible Markup Language (XML) – A universal format that allows for data transfer, especially for data on the web. Form controls – Graphical user interface objects (such as a text box, check box, drop-down lists or command button) that let the user control the program. The controls display data or choices, perform an action, or make the user interface easier to read.

5 5 Terms Used in This Lesson Form Field – A box or other designated location in a form where a particular type of data (such as name or address) is collected. Frames – Separates panes on a web page that help to organize the text and graphics. They can be resized and have borders. Schema – Provides the structure or the blueprint for the XML elements. Web Server – A computer that hosts Web pages and responds to requests from browsers.

6 6 Creating an Online Form Using Form Controls Word provides graphical user interface objects called form controls. (check box, text box, drop-down lists) An online form must be created as a template. Open the File menu, choose New, then select On My Computer, select the General tab, then choose Blank Document, click Template and OK.

7 7 The Forms Toolbar Includes: Text Form Field, Check Box Form Field, Drop-Down Form Field, Form Field Options, Draw Table, Insert Table, Insert Frame, Form Field Shading, Reset Form Fields, Protect Form.

8 8 Text form fields enable the user to enter text. Specify the format for text. A maximum length for the text entry. Use default entry that displays in the field until the user inserts new text. Text Form Fields

9 9 Protected Form Template Form fields all look the same and do not function until the form template is protected.

10 10 A drop-down form field restricts responses because the user must choose the options you make available. To create a drop-down form field click the Drop-Down Form Field option of the Forms toolbar Drop-Down Form Fields

11 11 The check box form field enables the user to select or clear an independent option. Choose the Check Box Form Field on the Forms toolbar. Check Box Form Fields

12 12 Help messages provide instructions for the user to complete the form. Click the Add Help Text button on the Form Field Options dialog box. Form Help Messages

13 13 Frames Toolbar Table of Contents in Frame Delete Frame Frame Properties You can use the Frame toolbar to add and remove frames in a Web document. You can also format the borders around the frame.

14 14 The Web Tools Toolbar Checkbox Textbox Text Area Drop-down box Submit Use the Web Tools toolbar to add form controls to a web page.

15 15 Time Saver You can use a shortcut in Word to create same fake text, and then later replace the fake text with the final text. =rand(3,5) “rand” means random “3” means number of paragraphs “5” means number of sentences

16 16 Summary Form fields enable you to quickly format forms for a variety of responses. An online form must be created as a template. Text form fields enable users to enter text for their responses. Drop-down list boxes restrict available choices.

17 17 Summary (cont.) Check boxes are positioned next to independent options that are to be selected or cleared. Help messages provide instructions for the user. You must protect the form template to prevent users from changing the content of the form.

18 18 Summary (cont.) Web pages opened in Word look like other Word documents with tables that organize the text and graphics. You can add or remove frames and format borders around frames. When you create a form for a Web page, you must use form controls that are standard HTML format for the World Wide Web.
