WHO WHAT WHE WHERE WHEN HOW WHY 1 2 3 4 The PA SBIRT Vision: Screening, Brief Intervention and Referral to Treatment (SBIRT) is a comprehensive, integrated, public health approach for early identification and intervention with patients whose patterns of alcohol and/or drug use put their health at risk.1 WHAT WHERE WHE Current PA SBIRT Project Sites Sharon Medical Group Family Medicine EMPOWER3 Center for Health Altoona Family Physicians UPMC Pregnancy Care Center 1 2 3 4 PA SBIRT aims to partner with healthcare sites to implement SBIRT services throughout the Commonwealth. WHEN Over the course of the 5-year project period, PA SBIRT project sites will receive personalized training and technical assistance to seamlessly integrate SBIRT services into the clinical workflow. EVIDENCE BASED IMPLEMENTATION FRAMEWORK PERSONALIZED CLINICAL TRAINING & PROTOCOL DEVELOPMENT PREMIER SBIRT TRAINING CURRICULUM HOW INTEGRATED CARE COORDINATION ACTIVITES PA has the 14th highest drug overdose mortality rate in the U.S. (15.3 per 100,000). 2 ELECTRONIC HEALTH RECORD INTEGRATION WHY Only 6% of adults seeking SUD treatment in PA had access within two weeks. 3 ROBUST CONTINUOUS QUALITY IMPROVEMENT PROCESS TIMELY AND ACCURATE DATA COLLECTION TOOLS The PA SBIRT Vision: Ensure that every patient in our primary care clinics receives the right screening, brief intervention, and referral to treatment (SBIRT) services by the right providers every time.
