Integrated Care Home Team Duane Patterson Team Manager

Background Then Now Peripatetic Team. Care Home Liaison Team. Older People’s Review Team. Integrated Care Home Team. Better partnership and joined up working. One point of contact for residents, family and care homes.

Peripatetic Nurses Consist of 5 Registered General Nurses. Extensive knowledge and experience in caring for frail older people living within care home settings. Provide expert nursing advice and training sessions to care home staff. Facilitate improvement of clinical skills and development of skills for unqualified staff through training and work shadowing within care homes. Signpost care home staff to access specialist nursing advice. To act as liaison between multidisciplinary teams, departments and other agencies.

Mental Health Nurses Consist of 5 Registered Mental Health Nurses. Direct link to consultant psychiatrist. Dedicated pharmacy support. Provide specialist assessment, treatment and review for older people experiencing mental health problems within care homes. Medication administration and review. Education and support to care homes in managing stress/distressed symptoms. Support transitional care from hospital to care home. Support care homes in reducing hospital admissions.

Review Officers Consists of 5 Review Officers (social workers). Annual review of resident outcomes and care home placement. Duty system in supporting residents, family and care homes between annual reviews. Local Authority guardianship role and supervision of private welfare guardians. Designated Council Officer to undertake Adult Support and Protection Inquiries within care homes. DWP appointee in managing resident benefits.

Support for the PEOLC Project Delivery of Palliative Care Course- train the trainers. Offer support to staff, using different tools such as SPICT and Abbey Pain Score. Advice and implementation of Just in Case medication. Symptom management i.e. breathlessness, coughing, pain, agitation/anxiety, nausea/vomiting. Administration of medication- support in use of butterfly needles and syringe driver. 

Support for the PEOLC Project Support with DNACPR and ACP.  Support residents, family and care home staff to recognise progression of Dementia. Ongoing support through the Dementia journey, which include medication review and strategies to manage stress and distress symptoms. Support to family and care home staff after resident’s death.
