Clearing the Path to Life After High School Inclusion Institute 2018: Answering the Call, Pathways to Success! Westwood Conference Center – Wausau, WI July 23-25th, 2018

Bios DAN KROHN MIKE HIPPLE is a Community Partnership Specialist at TMG. For the last 12 years, his work has been focused on partnering with people to build full lives in their communities using his expertise in Self-Directed Supports, Person Centered Thinking and home and community based supports. Dan has worked with TMG for 2 years using his talents to support people, families, schools, and service providers in successful navigation of the Wisconsin IRIS Waiver. He lives in Milwaukee with his wife and 3 year old son. MIKE HIPPLE Mike Hipple is an author, advocate, mentor and leader of the Wisconsin AAC Network. He has presented at conferences throughout Wisconsin regarding inclusion, self-direction, and communication strategies. He lives in Appleton, Wisconsin, where he is an active member of many community groups, including his local Kiwanis club. Mike is currently serving on the Wisconsin Council on Special Education.

Learning Objectives Demonstrate an understanding of the Principles of Self-Determination. Recognize the importance of natural supports and community connections. Understand how to support self-advocacy and self-direction in transition.

Who is TMG? Leader in community-based long term supports, quality management and service design for 30+ years. IRIS Consultant Agency (ICA) since 2008, partnering with over 13,500 people. Self-Directed Personal Care (SDPC) Oversight Agency supporting over 6,000 individuals who have chosen SDPC. Quality oversight for community-based services since 1986. Health and human services system analysis, design and training. Locally-based operations with 600 staff in communities throughout Wisconsin.

Clearing the Path Ahead Tips for Partnering with People: Develop valued roles and community connections. Identify short-term and long-term goals. Access local resources. Create individual support and services plans. Support responsible use of plan budgets (if enrolled in the IRIS program). Support people to find meaningful days through employment and community engagement.

Self-Determination & Self-Direction Self-Determination – A way of life An individual’s basic right to decide how to live her/his own life and be a full citizen. Self-Direction – A tool to support a self-determined life Maximizes choice and control for people who use long-term care services and supports. Values the role individuals and families play in knowing what works best for them. Promotes community connections, including employment, as a way for individuals to create valued roles and use unpaid and natural supports.

Principles of Self-Determination Freedom Authority Support to decide how to live one’s life. Authority over available resources, including people and a specified budget. Support to organize resources in ways that are enhancing and meaningful. Responsibility for the wise use of public dollars and giving back to the community. Confirmation of the important role that individuals play in a redesigned system and of the ability to control their own lives.
