How is reablement delivered? A short-stay accommodation based service – up to 6 weeks Designated self-contained furnished flats Based in a 24/7 staffed Extra Care setting Up to 20 hours of 1-1 support Delivered by a multi disciplinary team Directly accessed from the hospital or community Assessments on the ward Intensive, time limited and goal orientated Subtitle

The Journey Multi Disciplinary Team (MDT) Assessment Reablement potential and goals agreed Discharge planning by hosptial team Arrival and introduction at service Assisitve technology and final assessments completed 6 weeks of Reablement and therapy begins Weekly goal reviews with MDT Discharge to agreed destination 70% return home 30% other appropriate accommodation

Case Study - Jim Jim, 65, stroke, poor mobility and known to self neglect and isolate himself Admitted to hospital Not previously known to Adult Social Care 20 hour a week care package agreed upon discharge from hospital Reablement Goals Mobilise independently using zimmer frame Build strength on right hand side to enable 1 person manual handling Carry out personal care with encouragement Prepare simple meals with support Grab rails and toilet raiser to be installed at home Maintain regular contact with GP
