HEART The Home Emergency Alarm Response Team

2 What is our function ? The Home Emergency Alarm Response Team (HEART) aim to provide an emergency response service to individuals in difficulty via a community alarm service (Telecare) 24hours a day 365 days per year. The Service responds to people in an emergency situation who may have fallen or are in difficulty We work closely with other emergency service to ensure a comprehensive and co-ordinated response to vulnerable people The service contributes to the discharge pathway supporting timely hospital discharges to reduce hospital stays and to increase independence for individuals to live safely in their own home

Requests for response : November 2015: 969 November 2016: 1073 Number of initial installations: November 2015: 129 November 2016: 166

4 A&E avoidance – minimising YAS call outs
During Jan- May 2016 the HEART responders attended falls Of those an ambulance was called 51 times 5 referrals were made to the ECP service The others were maintained within their home
