Request for Disposition of Anatomic Specimens Complete the upper portion of the Request for Disposition of Anatomic Specimens form, which can be found at the OOAS website http://www.ooas.pitt.edu, or by email request at matusaksa@upmc.edu Please provide all information requested, as follows: Specimen Type: Identify the tissue type, e.g., cephalus, arm, upper/full torso, knee, pelvis/hemipelvis, etc. Quantity: Specify the number of specimens to be cremated Specimen source: Name of approved tissue supplier from which specimens were procured Contact person filling out the form Account number to be charged for cremation cost Contact person signature z

Request for Disposition of Anatomic Specimens Supporting documentation to be attached to each request: A copy of the shipment packing list or tissue distribution sheet that was included in the shipment by the tissue supplier (includes donor ID numbers, tissue type, quantity). A check mark must be placed beside each specimen included in the disposition request. If there is no supporting documentation (e.g., specimens were procured prior to 2007/2008), then a letter from the department chair must be provided authorizing the disposition.

Request for Specimen Transport Department of Parking, Transportation & Services If specimens will need to be transported to Scaife Hall for disposition, please make arrangements after you have submitted the Disposition Request to the OOAS. Contact: David Huey Phone: 412-977-6693 Email: dhuey@bc.pitt.edu PACKING INSTRUCTIONS: DO NOT include Solid Wastes: Such as absorbent material (pads and paper towels) and PPE (gloves, disposable lab coats, disposable face protection) DO NOT include sharps: (scalpels and needles) which must be collected separately in a Sharps container Specimens are to be placed in biohazard bags and twist tied closed. Place biohazard bag(s) into approved biological waste boxes Seal (top and bottom) with packing tape, and place Disposition label (pictured) on top of the box. Labels may be requested at matusaksa@upmc.edu Maximum box weight limit – 30 lbs. (based on Transportation Guidelines)

Biological Waste Disposal /Supplies Biological Waste Bags and Boxes Must use bags labeled with a biohazard symbol and are required to be red or orange in color Boxes should be lined with biological bags Purchase of the biological waste bags are the responsibility of the lab To obtain biological waste boxes, please contact EH&S at 412-624-9505 or by email at safety@ehs.pitt.edu
