Man dies after power lines accident in Orkney A man has died following an accident involving overhead power lines in Orkney. The incident happened during a lifting operation in St Mary's village on Sunday. Cliff Cooper, aged 32, was standing next to a telehandler vehicle when the jib of the machine touched overhead electricity cables. The driver - understood to be a family member - was unhurt in the incident.

POLICE have confirmed that a woman has died from injuries sustained in an industrial incident involving a forklift truck in Scalloway last month. Karen Allan, 61, from Scalloway, passed away at Aberdeen Royal Infirmary on Thursday evening. The incident occurred shortly after 11.40am on 31 January at Blacksness Pier. She was on foot at the time. She was taken to Lerwick's Gilbert Bain Hospital before being transferred to Aberdeen. Road policing senior investigating officer Sergeant Donnie Mackinnon said: "Our thoughts are with the family and friends of Mrs Allan at this time. "Police enquiries are ongoing in conjunction with the Health and Safety Executive to establish the full circumstances of the collision.

Statistics from 2012-17 show that the two major causes of fatalities in construction are falls from height & being struck by vehicles (25 people). Construction workers still 3x more likely to be killed at work than other sectors. Good news is that fatalities are down by a fifth since 1990. The majority of FLT accidents occur using counterbalance trucks, but the use of telehandlers is more widespread now & they’re  the work horse of small sites.

Underlying causes: lack vehicle/pedestrian segregation when driver vision is obstructed overlapping activities e.g. at a port or construction work in an occupied yard/premises reversing unaided poor planning of lifting ops & work at height (current tattie box investigation)

Underlying causes cont’d no training in use of lifting attachments design of temporary roadway e.g. slopes, speed, signs, visibility, space, one-way routes, buried or overhead services maintenance activities lack of maintenance selection of drivers & refresher training

Underlying causes cont’d operator error unauthorised use of FLT communication between driver & person in control of operation lack of or poor use of alarms, cameras/mirrors high-vis management of changes to work method - no review of RAMS

HSE investigations: HSL/Police do a 3D laser shot of the site & often use an HSL expert to show the driver's view.  Use of available CCTV & find out about 'custom & practice' for how the job is usually done. Work done by HSL usually costs at least £5k without including time for interviewing witnesses & report writing. We have 2 FLT investigations ongoing & 1 with CPFS in Inverness.
