Page 5 of 14 Well Re-Entry Tabs a.Admin - this tab contains Contact information and Technical Contact, if needed. b.Well Details - this tab displays the Well ID, Agreement, Formations, Terminating Location, and other Well Information for this CMA type.

Browse and select from your directory to attach this document. After saving, proceed with the rest of the Well Details tab. Complete Well Details Tab – Attach Letter of Explanation

Page 9 of Enter Crown Agreement Number 3. Click on Search 4. Select the checkbox 5. Click OK Ensure to SAVE after completing a search for a Crown agreement. 1. Click on the find button ( ) to search for a Crown agreement Complete Well Details Tab – Add Agreement Number

Page 10 of Enter Previous Approval Number if applicable 2. At least one Purpose and one Substance must be selected. An AER Order is mandatory if the purpose of Repair/Re-abandoned is selected. Browse and select a PDF file for the AER Order. Complete Well Details Tab – Well Details

Page 11 of Click on the Formation dropdown to select an option 2. Click Add 4. Click on Save 3. Select and Add a Terminating Formation from the dropdown Complete Well Details Tab - Formations The Terminating Formation must be selected if different from the (In the) Formation.
