A 38 year-old woman was admitted for a first-ever generalized seizure with eclampsia at 27 weeks of gestation. The eclampsia was associated with a reversible posterior encephalopathy. a | An axial MRI fluid attenuation inversion recovery (FLAIR) sequence and apparent diffusion coefficient (ADC) cartography showing bilateral symmetrical posterior hyperintense lesions with increased ADC, suggesting vasogenic oedema (arrows). No vessel irregularity was present (image not shown). The patient underwent a caesarean section, and was treated with intravenous magnesium, enalapril, nicardipine and furosemide. On day 11, the patient presented with a left middle cerebral artery stroke. b | An axial MRI diffusion-weighted imaging sequence performed 2 h after symptom onset with a large left hemispheric ischaemic stroke.
