EVERYTHING YOU NEED TO KNOW TO TRYOUT FOR A SPORT… FALL SPORTS TRYOUTS ARE AUGUST 1ST, 2017 This power point details the information needed to try out for WPHS sports. This and all forms are found under ‘files and links’. MAKE COPIES OF EVERYTHING FOR YOUR RECORDS! Yellow Card will be issued when the following requirements have been met: 1. VHSL physical form (new form has February 2017 in the upper right) 2. Emergency care form 3. Either private or school health insurance 4. On line concussion seminar (parent and student) 5. Weight room use form 6. All students must have passed 5 classes last semester (9th graders are exempt during the fall season)

YELLOW CARDS In order to ensure compliance with state and county eligibility requirements, WPHS has begun the use of eligibility cards (Yellow Cards) Students wishing to try out must bring all forms to the Student Activities Office NO ONE MAY PARTICIPATE WITHOUT A COMPLETED YELLOW CARD!

# 1: VHSL PHYSICAL FORM Three pages are filled out by parent, one page by doctor. Physical must have been performed AFTER May 1, 2017. Do NOT turn this in to ANYONE other than the Student Activities Office staff. Trust us..this is good advice. The student signs in two places, parent signs in three. Make sure Dr. has signed and DATED page 3. If you had your physical done at school, we will have that on record.

# 2: EMERGENCY CARE FORM BOTH sides must be completed and form must be signed by parent. Make LOTS of copies as you’ll need them for the main office and each sport you play. Bring a copy to Activities Office (even if you’ve already turned it into a coach).

# 3: HEALTH INSURANCE Students must have either private insurance or purchase school insurance. School Accident Insurance may be purchased prior to the start of the season. You can find the name and contact information of this year’s vendor on the website. This must be completed online.

SCHOOL HEALTH INSURANCE K&K Insurance will provide school accident insurance for the 2017-18 school year.  High Option School Time coverage is recommended for all sports except football, and all out-of-season practice and conditioning.  Football option is required for participation in Freshman, JV or Varsity football  August 1st through the end of the season ONLY.

# 4: CONCUSSION EDUCATION PROGRAM Student and one parent must complete this online seminar. You do not need to print anything when you are done as your information automatically goes into our system. DO NOT CONFUSE THIS WITH THE IMPACT PROGRAM used by our Certified Athletic Trainers to measure baselines on each athlete. http://www.fcps.edu/supt/activities/atp/health/concussion/ceov_program.shtml

CONCUSSION EDUCATION PROGRAM… Virginia Code 22.1-271.5 requires all student-athletes and a parent complete a concussion education program EACH SCHOOL YEAR. Parents need only complete the program one time for all their children participating in FCPS athletics if they register each child's FCPS ID number PRIOR to viewing the slideshow. Your computer must allow "pop-ups", if you have a pop-up blocker enabled, please "allow pop-ups" for this application.
