2016 Annual Enrollment Period: October 26 – November 10, 2015 Action Required

Action required or pay more in 2016 Tobacco User Surcharge New for 2016: $300 (Individual) & $600 maximum (Family) Annual Surcharge (Default) You Must Take Action to Avoid the Surcharge Non-tobacco users: Certify that you, your covered spouse or domestic partner and/or children age 19 and over are not tobacco users Tobacco users: Enroll in a tobacco cessation program by December 31, 2015 ! New for 2016: Group Universal Life Insurance Higher premiums will be applied should you use tobacco or fail to attest. Tobacco user rates will also apply to spouse or domestic partner coverage under Dependent Term Life Insurance

Avoid the tobacco use surcharge Tobacco products: Cigarettes Smokeless tobacco Cigars Electronic cigarettes Pipes Tobacco cessation programs WebMD Tobacco Cessation Program Prudential Health Clinic Fresh Start Program Check with your health care program carrier Tobacco cessation program of your choice At no cost to you May provide discounted or free programs At your own cost

What’s changing for 2016 Tobacco surcharge applied to your medical contributions will increase to $300. Also, an additional $300 surcharge that will be applied if any of your covered dependents age 19 and over use tobacco Tobacco usage, or failure to attest of non-usage will result in higher premiums for Group Universal Life Insurance and Dependent Term Life Insurance High Deductible Health Program In-Network Out-of-pocket Maximum for family coverage is decreasing from $8,000 to $6,850 Effective February 20, 2016, the Cigna HSA will be administered by HSA bank instead of JPMorgan Chase Additional losses will be covered for Business Travel Accident and Accidental Death and Dismemberment Due to the continued rising cost of the health care, your contributions may be higher; however, the aggregate cost share is not changing and will remain at 72%/28% for 2016

What’s changing for 2016 Prescription Drug Changes administered by Express Scripts Unless physician required brand name for a certain medication, participants will be responsible for the difference between brand name and generic price if generic exist Specialty drugs will be required to be filled through Accredo, Express Scripts’ specialty pharmacy Aetna and Cigna medical program participants who are age 18 and older may receive vaccines for a variety of conditions at no charge when received at an Express Scripts participating pharmacy Long Term Disability benefit of 50% of monthly eligible earnings up to a new maximum of $12,500 per month You can elect Optional Long Term Disability and pay for an increase in your benefits to 65% of your monthly eligible to reach a new monthly benefit maximum of $25,000 per month for grades 7P-18P, and $40,000 per month for grades 1P-6P

Dependent Care Reimbursement Account Annual Maximum for 2016 Action required: Flexible Spending Accounts Health Care and Dependent Care Reimbursement Accounts ! You must make an election for 2016 if you want to participate. 2015 elections will not carry over. Health Care Reimbursement Account (HCRA) and Limited Purpose HCRA Annual Maximum for 2016 $2,550 Dependent Care Reimbursement Account Annual Maximum for 2016 $5,000 (Includes Prudential match of 25% up to $1,000)

Flexible Spending Account Estimator – help you determine how much you might want to contribute to a Health Care of Dependent Care Reimbursement Account Life Insurance Needs Estimator – helps you asses whether you have the right amount of life insurance Disability Estimator – helps you calculate how much disability coverage you might need

Smart choices, healthy lives Cigna and Aetna cost transparency tools: Show how significant the cost of a test can be from one provider to another for the same procedure in the same area Help you compare non-emergency procedures so you can spend your health care dollars effectively Hand X-ray in Northern NJ: $26 – $568 MRI of the leg in Greater Philadelphia: $400 – $3,700 The tools also provide information about the quality of providers in the networks.
