1 2018 Employee Benefits In addition to todays presentation:
Employees may all access a 6 min video that will provide the an overview of benefits and a Benefits Guide that will provide detailed benefits and cost information for all plan benefits. Todays presentation is primarily focused on explaining the changes of the medical plan to assure all employees understand the plan options available and the employee’s responsibility during this open enrollment period. Because a lot of questions may be answered on the next slide of this presentation. We ask all employees to hold their questions until the Q&A period after each section of this presentation.

2 Medical & Pharmacy Effective March 1, 2018

3 Medical & Pharmacy Blue Cross Blue Shield will remain the insurance carrier through February 28th, 2018. All employees will receive new ID Cards to begin using for prescription and medical services received on and after March 1, 2018. No changes to the medical services that are covered under the medical plan. Possible Changes to the prescription drug formulary, medical precertification requirements and network providers.

7 HSA Health Savings Account
A Health Savings Account (HSA) allows individuals enrolled in a high deductible health plan to save money tax-free to pay for eligible medical expenses. Eligible Medical Expenses Include: Doctor’s office visits and procedures Diagnostic devices (such as blood sugar test kits for diabetics) Prescription Drugs Eye surgery (such as laser eye surgery or radial keratotomy) Wheel Chairs COBRA Premiums Contribution limits: $3,450 individual/$6,900 family (2018) Catch-up contributions (age 55 or older) = $1,000 Tax Benefits All funds that go into and come out of an HSA are tax-free (as long as funds are used to pay for qualified medical expenses). Contributions to HSAs are not subject to federal income taxes. Earnings to an HSA from interest and investments are tax-free. 3.Distributions from an HSA to pay for qualified medical expenses are tax-free. 401 K you don’t pay taxes when you put money in but you pay when you take it out. An HSA you don’t pay when you put it in and you don’t pay when you take it out if you use it for qualified medical expenses. When you file your taxes each year. You list how many HSA dollars you used for Qualified Medical expenses if you used all for qualified medical expenses you do not pay any taxes. If you decide to take it out and use it for a vacation or to buy a boat. When you file your taxes and cannot show the money was spent for qualified medical expenses you will have to pay taxes on that money and if under age 59 will an additional 20% penalty. Again similar to a 401K plan Any unused portion will rollover year to year. If you leave the company, you may take it with you.

8 HSA Health Spending Account Central Control, LLC will Contribute:
A HSA allows individuals to set aside money tax-free to pay eligible medical expenses. It can also be used to pay for prescriptions, dental, and other qualified medical expenses. The HSA funds you contribute are deducted on a pre-tax basis. Central Control, LLC will Contribute: $400 Single Coverage $700 Family Coverage Company Contribution is deposited on March 1, 2018 Employees with current HSA accounts through TASC will use their current Debit cards until $$ are exhausted. Only available with election of the HDHP Employees must elect to open a HSA account in the ADP system

If an employee contributed $50 a month to HSA and used those dollars to pay for the some of the $6617 – with the tax savings the total cost would have been 11,040

Spousal Surcharge Beginning in March, employees will pay an additional flat dollar amount each month for health coverage if the employee enrolls their spouse who is eligible for coverage through their own employer. 15 Life Insurance Voluntary Life & AD&D Coverage
Carrier change from Unum to Symetra. Central Control, LLC provides Basic Term Life and AD&D insurance at no cost for its employees working a minimum of 30 hours per week. Voluntary Life & AD&D Coverage Employee – 5x Salary Maximum of $500,000* Guarantee Issue Amount $200,000 Spouse – Up to $250,000* Guarantee Issue Amount $25,000 Dependent Children - $10,000 Maximum *Evidence of good health may be required

16 Carrier change from Unum to Symetra.
Disability Carrier change from Unum to Symetra. Central Control, LLC offers Short Term Disability and Long Term Disability through the voluntary group plan. Long Term Disability Long Term Disability benefits begin 90 days following the disabling injury or illness. The benefit pays 60% of your monthly salary ($5,000 maximum). Benefits are payable to Social Security Normal Retirement Age. Short Term Disability Short Term Disability benefits begin on the 15th day following the disabling injury or illness. The benefit pays 60% of your weekly salary ($1,000 maximum) for up to 11 weeks.

17 Employees will remain in HDHP without any HSA Account or Contribution
Open Enrollment Log In to Your Web Site After you log in, use the Enrollments page to review your current benefits, if applicable, and make your Open Enrollment selections. Employees that do not login will be locked into benefits without the ability to change until February 28, 2019. Employees will remain in HDHP without any HSA Account or Contribution Mandatory

18 Deadline to make elections:
Open Enrollment Benefits become effective March 1, 2018 and your selections will remain in effect until February 28, 2019. Deadline to make elections: January 19, 2018
