FAA Airports Division (ACSI): once the report is received, the FAA Airport ACSI is responsible for the Investigation of the VPD and will notify the Airport Owner of the incident through a Letter of Investigation (LOI).

NOTIFICATION PROCEDURES - cont Airport Operator : Responds to the Letter of Investigation to explain the circumstances that led up to the incident and what actions the airport is taking to prevent any further incidents. FAA Airports Division: Reviews the airport’s response and makes the final determination as to whether the airport has taken the appropriate action. If the airport’s response is satisfactory, the LOI is closed out with the airport. FAA From 8020-25 is used as the final report for ATCT’s Preliminary Report and the Incident is closed. An Unsatisfactory Response or No Response from the airport operator may result in further action being taken by the FAA.

NOTIFICATION PROCEDURES - cont Items reviewed by the ACSI may also include the airport’s: Airport Certification Manual (ACM) Airport’s Training Program Airfield Signage & Marking Plan Airport Lighting Configurations Airport Condition Reports (NOTAM) If assistance is required, the airport may be referred to the FAA Runway Safety Office or agencies for the appropriate guidance or training.
