Patients Medical Record System (PMRS) A medical record, health record, or medical chart is a systematic documentation of a patient's medical history and care. The term 'Medical record' is used both for the physical folder for each individual patient and for the body of information which comprises the total of each patient's health history.

Patients Medical Record System (PMRS) How do medical records being kept traditionally? Traditionally, medical records have been written on paper and kept in folders. These folders are typically divided into useful sections, with new information added to each section chronologically as the patient experiences new medical issues. Active records are usually housed at the clinical site, but older records (e.g., those of the deceased) are often kept in separate facilities.

Patients Medical Record System (PMRS) PMRS is a system which contains: patient's identification information patient's health history (what the patient tells the health-care providers about his or her past and present health status) patient's medical examination findings (what the health-care providers observe when the patient is examined) Other information may include lab test results; medications prescribed; referrals ordered to health-care providers; educational materials provided; and what plans there are for further care

Purpose of PMRS The information contained in the medical record allows health care providers to provide continuity of care to individual patients. The medical record also serves as: a basis for planning patient care, documenting communication between the health care provider and any other health professional contributing to the patient's care, assisting in protecting the legal interest of the patient and the health care providers responsible for the patient's care, and documenting the care and services provided to the patient. document to educate medical students/resident physicians, provide data for medical research.

Drug Information Systems A computer-based system where information on all prescription drugs dispensed to customers will be gathered and securely stored. It is electronically linked to all health care sites, such as: pharmacies physicians' offices health centres addiction centres hospital pharmacies Emergency Departments

Drug Information Systems It is accessible to authorized health care professionals, including physicians and pharmacists. All pharmacists are required by the Pharmaceutical Information Act to record all prescriptions filled with the DIS.

Drug Information Systems Benefits: Reduce the potential for harmful drug interactions, prescription errors, and adverse drug reactions. Make it easier for pharmacists to report adverse drug reactions to Health Ministry. Help pharmacists to counsel patients about their medications. Promote better patient care by enhancing the partnership between physicians and pharmacists. Eliminate the need to repeat your medication history at each visit to your physician or pharmacist.

Drug Information Database A database that is created to store data in drug information system. The database contains: Information on thousands of prescription over-the-counter medications potential side effects and interactions with food and/or other drugs.

Drug Information Database A few examples of drug information database: Drug InfoNet provides drug and disease information for your healthcare needs. http://www.druginfonet.com/ This database stored top 200 prescribed drugs listing, searchable database, by action, interaction, brand name and imprint code. http://www.rxlist.com/script/main/hp.asp This website provides drug information such as: US Brand Names, descriptions, indications for use, warnings regarding a drug's use, etc. You can also search for supplementals such as herbs, vitamins, etc. http://www.mayoclinic.com/health/drug-information/DrugHerbIndex/UPDATEAPP=false&FLUSHCACHE=0
