The bigger providers systems in the area. We also offer a PPO that has a little larger network H1716-006 Mid Missouri: (Benton, Boone, Cole, Cooper, Howard) $19 Premium $15 PCP $45 Spec $6400 MOOP $330 1-5 Hospital Copay $285 Outpatient Surgery Copay In this HMO service area we do have the University of Missouri system and we have Boone County hospital in-network as well. Kansas City includes: (Clay, Cass, Henry, Jackson, Lafayette, Platte, Pettis,  Johnson, Ray Counties in Missouri) Lead product is the HMO H2649-004: $25 premium  $10 PCP $45 Spec   $4000 MOOP  $275 1-6  Hospital copay $275 Outpatient Surgery copay There is also a H2649-012 HMO MA only and an H2649-024 $0 HMO with very skinny benefits. For Kansas City we do have  large network of providers in the HMO. We also offer a LPPO H1716-001 that that has an even larger network to choose from. New for 2014 the KC Metro in (Clay, Cass, Jackson and Platte Counties) is the H5416-019 Dual Eligible SNP plan. Springfield, MO: (Barry, Cedar, Christian, Dade, Dallas, Douglas, Greene, Howell, Jasper, Laclede, Lawrence, McDonald, Newton, Ozark, Polk, Pulaski, St. Clair, Stone, Taney, Webster and Wright) Lead product is the HMO H2649-020: $0 Premium $15 PCP $45 Spec $4900 MOOP $225 1-7 $175 Outpatient Surgery Copay   We also offer a Dual SNP in all the above counties except for Ozark and Howell counties. H5416-019 We also offer a Chronic SNP currently in Christian and Green counties, but we will be expanding this service are to (Barry, Dade, Dallas, Douglas, Jasper, Laclede, Lawrence, Newton, Polk, Pulaski, Stone, Webster and Wright) H2649-025 this is a Diabetic SNP with Richer benefits than our competitors. We do offer a LPPO H1716-006 which has an even larger provider network, including COX which is the other large system in the area.
