Improving health status of communities Goal: To promote effective prevention & treatment of chronic disease: Task 1: To improve cardiac health and reduce cardiac healthcare disparities: (ABCS) Task 2: Reducing disparities in diabetes care: Everyone with diabetes counts (EDC)

Better healthcare for communities: safe care Beneficiary-centered, reliable, accessible Task 1: reducing healthcare associated infections in hospitals-MRSA/VRSA/CDI/SSI/CRE Task 2: reducing healthcare acquired conditions in nursing homes-6% composite score or lower on 13 HAC measure set-75% recruitment goal Task 3: coordination of care-reduce readmissions by 20% > community based coalitions (CBO).

Strengthen Survey Process Standards and Enforcement The QIN-QIO shall support the adoption and utilization of the Division of Nursing Homes’ Quality Assurance Performance Improvement (QAPI) as a framework for nursing homes participating in any National Nursing Home Quality Care Collaborative (NNHQCC).

Better care at lower cost Goal: to make care more affordable Task: Value-based payment (VBP) to >by 15% nationwide, Quality reporting (IQR/OQR)in IPPS/CAH/PCH/ASC/IPFs Physician quality reporting system (PQRS) with >100% in reporting and 50% with significant improvement in outcomes
