Southeast Community Health Center The City of Fort Worth partnered with a local non-profit, North Texas Area Community Health Centers who owns and operates the Southeast Community Health Center The Health Center opened May 15, 2014 and is easily accessible by the health consumers who need it most. With approximately 20 employees, including 3 doctors, an estimated 4,200 patient visits per doctor are conducted annually. The location of this health center and the healthcare that is provided offer services that low- to moderate income citizens might not otherwise have access to. This facility reduces strain on already overburdened local hospitals by seeing some of the higher-risk patients when pain or illness is not as severe and before they resort to visiting more expensive offices that offer shorter wait times or they incur large emergency room bills.

Southeast Community Health Center NTACHC NTACHC is a Federal Tort Claims Act-designated facility, which leads to savings of millions of dollars in malpractice insurance premiums for NTACHC. Savings are invested into additional services for the community. They provide care to anyone at the Southeast Community Health Center regardless of income, employment, health insurance coverage, or ability to pay for services. Diabetes management includes assistance such as physician screenings, training classes, and one-on-one counseling. Space dedicated for dental clinic.

Southeast Community Health Center Services Services include well-woman exams and adult physicals, family planning services, maternity care services, childhood immunization and physicals, adolescent health services and preventive care for children and adults. Other services include routine examinations and management of chronic illness such as diabetes, hypertension, and asthma.

Southeast Community Health Center Funding CDBG funds were used to construct the building but when the obligations for these funds were unable to be met by the nonprofit originally designated for the construction, they opted to repay the city and we received the land and building. Original non-profit developer received a $1.25 million grant as part of the American Recovery and Reinvestment Act (ARRA) of 2009 For the development of the Health Center, the city sold the land with the building to NTACHC. Additionally, the city provided $170,231 in CDBG funds to NTACHC to finish out the interior. Total CDBG development costs for the construction and finish-out were $520,231. NTACHC conducted a capital campaign and contributed $306,590 Total cost of investment (including CDBG) is $826,821.00 ($306,590 NTACHC capital campaign) City also sold the building and the land to NTACHC for $67,600.00, but immediately put back the program income in for the completion of the finish-out. Sold adjacent property for $100.
