Pathway for Adults (P4A) Mental Health Service in Somerset Somerset Services – Reconnect Floating Support GOAL = support people to continue living independently and safely in their own homes for as long as possible and recover a better quality of life Since 2008 Reconnect Floating Support Service has provided housing related support to individuals living in their own homes with a diagnosis of dementia or over 65 with memory problems Our highly trained team support up to 70 people per week, working closely with other agencies, family and friends Support is tailored to the needs of the individual using a person-centred recovery, support and safety plan We recognise the importance of involving our clients, and their carers in the running and development of the service

Person-centred support for clients – empowering for independent living Information and advice, including benefit and training Mental Health support Support to develop skills Support for independent living Accommodation solutions and choices Group support Peer support Self-management programmes Crisis Solutions / Alternatives to Admission Self Harm Support Employment Opportunities Carers’ Support and Siblings Network Partnership Working Signposting to other agencies / groups Training: Mental health, Resilience, Living Independently, developing skills Volunteer opportunities: training and support Transport assistance Relaxation interventions for those experiencing anxiety Immediate support to clients with physical health problems that require medical equipment, aids & adaptations

Pathway for Adults (P4A) Mental Health Service in Somerset Our stepped support approach Our service delivery focuses on support that is flexible and can meet the changing client needs. High: 10+ hours a support a week. Individuals requiring this level of support long term will require this support to be purchased through individual budgets Medium: hours support a week Low: up to 5 hours supports a week
