Objectives 2 Discuss the mental and behavioral health issues impacting your patients, and how those issues impact you Decrease stigmas associated with mental and behavioral health Review language and tools to help the team care for patients with mental and behavioral health issues

Purpose for this Presentation 3 ACT (Advancing Care Together): 11 sites funded across the State integrating mental and behavioral health with primary care Message is clear – when you start the integration process, the team will be forced to engage with patients in unexpected ways Patients define who’s on their ‘care team,’ not practice members Team members need support if they are being asked to help care for patients with mental and behavioral problems

Behavioral Health: What Is It? 4 Behavioral health: relation between human behavior, and the well-being of the body, mind, and spirit Behavioral healthcare: services for people at risk of or suffering from, mental, behavioral or addictive disorders Behavioral health refers to mental health, psychiatric, marriage and family counseling, and addictions treatment; and includes services provided by social workers, counselors, psychiatrists, neurologists, physicians, psychiatric and mental health nurses
