Mandated Student Insurance Eligibility File Student’s information must be included on insurance eligibility file in order to access Enrollment form or Waiver form The following are required for inclusion on eligibility file: –Registered for classes for Fall 2016 –Graduate assistants set up in payroll for fall semester –Must have U.S. based mailing address on file with UGA –Must have a UGA address –Must report your gender to UGA – required by insurance

Enrollment Automatic Enrollment Process –Occurs late September for fall coverage –Occurs late February for spring/summer coverage Complete the Enrollment Form to activate coverage sooner! –Enrollment form link available at Select Mandatory Plan, see Enroll section –To access form, enter date of birth and student ID (81x#)

Dependent Enrollment Can purchase coverage for eligible dependents –legal spouse –dependent children under 26 years of age Complete Enrollment Form to activate student coverage first Enroll dependents directly with United Healthcare –Link on Enrollment Form confirmation page Pay dependent premiums directly to UHCSR

Coverage Periods Fall 2016 Coverage effective August 1, 2016 – December 31, 2016 Spring/Summer 2017 Coverage effective January 1, 2017 – July 31, 2017 Enrollment is by coverage period (not month-by-month) Cannot cancel coverage early

Waiver Requests Do you want to opt-out of the mandatory plan? Other coverage must meet certain minimum requirements Submit a waiver request each waiver period Fall waivers apply to the period of 8/1 – 12/31 Spring/Summer waivers apply to the period of 1/1 – 7/31 Visit (select Students, then Mandatory Plan Waiver) Waiver requests must be submitted by the September 5, 2016 deadline – no exceptions!

Waiver Request – Random Audit Waiver requests may be randomly audited during the semester. –You will be contacted via your UGA –Must provide requested documentation about other health insurance. Documentation must be in English. –Charges for mandatory plan may be assessed if requested information is not provided.

How Do I Pay Premiums? Student Account Charge –Undergraduate international students –Graduate international students who do not have a graduate assistantship –Doctor of Pharmacy students Payroll Deductions –Graduate students with a qualified graduate assistantship, fellowship or training grant Fellowship or training grant must be paid through UGA payroll to be eligible

Premiums for : Student Account Charge on Student Account International undergraduate students with a J or F visa type; International graduate students (J or F visa type) without a graduate assistantship; Doctor of Pharmacy students Fall 2016Spring/Summer 2017 $870 $1,206 *Approved waivers: Insurance charge removed from UGA Student Account within seven (7) business days from date of waiver approval.
