All rights reserved. 4.2 Entering New Patient Information (continued) 4-9 A chart number is a unique number that identifies a patient. –It links together all patient information stored in different databases. –It is an eight-character number. –No special characters such as hyphens, periods, or spaces are allowed. –No two chart numbers can be the same. –The system will assign a number when the chart number box is left blank.

A guarantor is an individual who may not be a patient of the practice but who is financially responsible for a patient account.
