Presentation transcript:

I found out why I was losing blood and losing weight. Helped my mind after my son died. I have had the most kind people care- they were determined to find out why my hemoglobin was so low. I can’t say enough about how wonderful these people are. They ROCK because they truly care! The mind, body, soul and heart. This clinic is awesome! When I didn’t have health care they allowed me to see doctors for just about everything that ailed me. It’s very difficult to find health insurance when you’re a single mom and working, but not have enough money to see a doctor. VIM is a God send for me and many others like me. They help with medications, labs, and anything else I have needed. The doctors and staff have been wonderful. It has been a challenge for me to maintain full-time status due to my desire to continue my education. I hold a few part time jobs but I haven’t been able to afford health insurance. Last year I became ill and missed work. I am so grateful to the services of VIM has provided. It’s amazing to receive excellent care from the moment you call to make an appointment, to the professional health care received. “Bed side” manner has not been matched for me. I will be working full time before the year is over and I will miss volunteers in Medicine when I am insured. May God to bless all involved in keeping the facility open. Volunteers in Medicine is a teaching facility. Through partnerships with the University of North Florida and St. Vincent’s, we offer hands- on experience and training alongside some of northeast Florida’s most accomplished and esteemed medical practitioners. Volunteers in Medicine is proud to partner and receive support from the Jacksonville medical community: Baptist Health St. Vincent’s Mayo Clinic University of North Florida Brooks College of Health Florida State College of Jacksonville’s Ophthalmic Technicians Consolidated Labs First Coast Cardiovascular Institute and more. OUR COMMUNITY PARTNERS Health screenings for chronic diseases like high blood pressure and diabetes Cancer Screenings, including mammograms Well Woman exams Vision screenings and prescription eyeglasses Medications Lab tests, including specialty labs Mental Health Counseling Education: Nutrition, Diabetes, and Weight Management Specialties: Dermatology, Endocrinology, ENT, Gastroenterology, Gynecology, Hematology/Oncology, Neurology, Ophthalmology, Orthopedics, Podiatry, Pulmonology, Rheumatology, Urology, limited Dental OUR FREE MEDICAL SERVICES VIM has helped me obtain much needed health care after being laid off from my job. I am a caregiver and have been taking care of my brother for 4 years. In order for me to continue helping him, I must take care of my own health. VIM allows me to do just that! VIM is a blessing to me! Thank you. I would have no health care if it wasn’t for you. You are a blessing. As a new patient, Volunteers in Medicine has helped me to obtain a primary physician so that I don’t have to wait until I’m really sick to go to the Emergency Room. They are very thorough and kind when providing service. It is very helpful to have a clinic like this. 41 E Duval St., Jacksonville, FL (904)  OUR PATIENTS’ GRATITUDE VIM has helped me and my family greatly. I am a blue collar worker with a family that could not afford health insurance for me and my family. Someone referred my wife to VIM and that is how we came to VIM. In today’s world, we still couldn’t afford to pay, so thank you to all the nurses, doctors, specialists, administrators, coders for giving your precious time to serve the sick. When I was sick, I couldn’t afford to go to the doctor. VIM was there to take care of me. Thanks VIM- you’re the best! I don’t make enough money to buy insurance. VIM has not only gotten my blood pressure under control and found other issues, but a variety of services that I otherwise could not afford. They have helped me maintain my health. I would like to thank everyone for their time that they donate, and the helpful and cheerful service they have given.

OUR PATIENTS Our patients work at least 20 hours a week and have an annual income between $13K and 250% of the Federal Poverty level (e.g. $29.7K for a single individual). These individuals either; make too much to qualify for Medicaid; their employers do not offer coverage; they cannot afford their employer’s group health insurance; or even if they qualify for coverage through the Affordable Care Act, they simply cannot afford the co-pays and costly deductibles. 5,144 Appointments for Patients 230 Volunteer Professionals 18 Medical Specialties $1.1M Volunteer Services OUR 2015 OUTCOMES 3,276 Medications Dispensed OUR IMPACT As a result of our unique volunteer model, Volunteers in Medicine is able to deliver at least $5 in services for every $1 donated. That’s an incredible Return on Investment! OUR MODEL Our success can be directly attributed to the passion and commitment of more than 230 ACTIVE VOLUNTEER medical and administrative professionals that perform a wide array of services at our downtown clinic five days a week. These volunteer professionals qualify patients, manage medical records and deliver vital preventative medical care to our patients. On-site program specialties include mental health counseling, vision and eye care, cardiology/heart disease prevention and maintenance, diabetes and nutrition counseling. Volunteers in Medicine’s signature “Healthy Habits” programs include regular patient wellness events and activities including blood pressure and glucose screenings, mammograms, stress tests, and “Healthy Habits” neighborhood walks. OUR MISSION Providing FREE outpatient primary and specialty medical services to Northeast Florida’s WORKING, low-income, uninsured individuals and their families to keep them healthy, employed and out of hospital emergency rooms FINANCIAL ACTIVITY PATIENT DEMOGRAPHICS Sponsored by Volunteers in Medicine Jacksonville, Inc. and the State of Florida, Agency for Health Care Administration
