Both preeclampsia and ROP are associated with impaired angiogenesis. However, the relationship between maternal preeclampsia and ROP is still controversial. We analyzed the association between preeclampsia and ROP in a Taiwanese cohort of very-low- birth-weight (VLBW) premature infants. Method Total 8652 VLBW infants were registered to the database of Premature Baby Foundation of Taiwan during 1997 to The exclusion criteria: 1. congenital or chromosome anomalies 2. those died before 36 weeks postconceptional age 3. those ROP status were unclear. We used polytomous logistic regression analysis to estimated odds ratio (OR) and 95% confidence interval (CI) for preeclampsia in association with ROP and severe ROP after adjustment for other risk factors. Severe ROP was defined as any stage 3 disease with plus disease, and stage 4 or 5 disease. The overall incidence of mild and severe ROP was 36.0 % and 12.2 %, respectively. Infants with maternal preeclampsia have higher maternal age and gestational age (GA), higher incidence of Cesarean section (CS), singleton, Apgar score >7 at 5minutes and small for gestational age (SGA), but lower incidence of patent ductus arteriosus (PDA), respiratory distress syndrome (RDS), sepsis, transfusion, male sex and ROP. In the univariate analysis, lower GA and lower birth weight, vaginal delivery, male sex, non-SGA, RDS, PDA, sepsis, transfusion, lower Apgar score at 5 minutes and without maternal preeclampsia were associated with both mild and severe ROP development. Factors associated with Preeclampsia and ROP, respectively Multivariate polytomous logistic analysis Multivariate-adjusted OR (95% CI) of developing any-stage ROP and severe ROP for maternal preeclampsia were 1.02 ( ) and 0.97 ( ), respectively. The results remain unchanged in stratified analyses according to SGA status.
