South Eastern Health & Social Care Trust Adult Disability Services

SEHSCT Adult Disability Health & Wellbeing Forum The promotion of health and wellbeing and reduction in health inequalities is a key priority in the South Eastern Trust. The partnership between the Disability Directorate and the Health Development Team seeks to ensure a coordinated multi-sectoral approach is maintained in addressing the specific needs of individuals with a Disability and to support the groups and communities who support them.

Adult Disability Services Health Development Plan The Adult Disability Health Development Plan has been developed to meet the health development needs of people with a: Learning Disability Physical Disability Sensory Impairment Neurodisability The plan has been co-ordinated in conjunction with the Trust Health Development Department in context to relevant regional strategy action plans. The Health Development Plan will be delivered to individuals and groups of people residing in a range of community living options and with people who are attending a range of day opportunity services.

Theme 2: - Ready for Adult Life “Equipped Throughout Life” takes account of particular needs across the life course and covers childhood and adulthood, with emphasis given to children and young people, and to supporting individuals’ transitions into and through adulthood and older age.

Learning Disability Service Framework Standard 15 People with a learning disability should be supported to have meaningful relationships, which may include marriage and individual, unique, sexual expression within the law, balancing their rights with responsibilities.

Standard 15 Contd… 1. Regional guidelines on sexuality and personal relationships are developed to ensure a consistent approach. 2. Trusts to facilitate appropriate training for staff 3. Trusts to facilitate appropriate training for service users and family carers 4. Increase in the number of people with a learning disability accessing sexual health and reproductive healthcare services

Each section of Part Two is organised with a summary of the protocol statement appearing first, followed by guidelines for implementation. 1.Personal, social and emotional development 2.
