The Transformation Journey As A Medical Home
Presentation transcript:

Background The Patient Centered Medical Home (PCMH) has become the framework for the future of primary care and the healthcare system in the United States. 1-3 It provides more patient centered care at a lower cost. Medical students do not currently have adequate knowledge and skills to function in a patient centered medical home. 4 There is a need for a learner-centered curriculum to provide students a realistic view of their current and future role in the healthcare system regardless of what specialty they choose. 4-5 The community setting is an ideal location for this education as it will provide benefit to the student, patient, and the practice. A PCMH curriculum was developed to address this need. Jamie Padmore, MSc and Peggy Weissinger, EdD Melly Goodell, MD, Chair, Department of Family Medicine Research Question Does the PCMH curriculum improve knowledge and attitudes about patient-centered primary care by 3 rd and 4 th year medical students? Methods Post-rotation survey 37 questions Anonymously reassess knowledge, and attitudes about PCMH Includes validated instrument, Patient-Practitioner Orientation Scale (PPOS), to assess “patient-centeredness.” 6 Required PCMH Curriculum for all 3 rd and 4 th year medical students Goal: increase knowledge, experience, and comfort discussing the PCMH and to understand the importance of primary care in the healthcare system. -PowerPoint presentation from the American Academy of Family Physicians -2 projects from menu of options -Reflection of an experience during their rotation Pre-rotation survey 40 questions Anonymously assess demographics, knowledge, and attitudes about PCMH Includes validated instrument used to assess “patient-centeredness.” 6 Q3. I understand the PCMH model. Q4. I am confident in my knowledge of the PCMH. Q5. I can educate others about the PCMH. Q6. The PCMH is an important part of the U.S healthcare system. Q7. The PCMH will play a role in my future career. Q8. It is important that I learn about the PCMH before I graduate. More education is needed to prepare medical students for future practice in a healthcare system organized around the PCMH. Conclusions PCMH curriculum did improve: 1.Student awareness of the PCMH. 2.Student understanding, confidence in knowledge, and perceived ability to educate others about the PCMH. 3.Student understanding of principles of coordinated care, enhanced access, and appropriate payment (not shown.) PCMH curriculum did not improve: Student understanding that primary care physicians do NOT act as gatekeepers of access to specialty care. PCMH curriculum did not change: 1.Student feeling that the PCMH is an important part of their education and the health care system. 2.Students’ correct understanding of principles of personal physician, physician-directed practice, whole-person orientation, and integrated care. 3.Total, sharing, or caring PPOS scores which measure “patient- centeredness.” 4.Student residency interests. Most students chose projects for curriculum with nursing home, home visits, and behavioral health clinic.
