What difference will the BCF make? “ To provide integrated services and pathways of care that help people to stay at home, avoid hospital admission and remain independent for as long as possible.” To put Sam’s story into practice closer integration of health and social care services to – improve the “service offer” and outcomes for people using pooled budgets to facilitate this to provide a more streamlined way of working for our employees – and opportunities for new roles continuous change has been in place for some time and will continue – to support and facilitate the drive for service improvement To manage increasing demand on services To deliver on national performance measures

BCF Overview Two main schemes: – Multi-Disciplinary Service – Dementia pathways and care Five Enablers: – 7 day working – Joint assessments – Digital Health care – Narrowing Health Inequalities – ICT systems and data sharing Plus Communications and Engagement

BCF Funding Not new money – Combination of existing – Plus top sliced NHS funding – Targets: Reduce Emergency Admissions Reduction in number of permanent admissions to Care and Nursing Residential Manage growth in demand across Health and Social Care No services will be commissioned from BCF unless they deliver against the BCF targets

BCF Funding Priorities are: – VCSE Health Initiatives (Social Prescribing) – Existing funding for services but a new funding mechanism (previously S256, not S75) – Protecting social care – Specific grants (such as Disability Facilities Grant) – New Care Act burdens (such as Carers) – Plus: MDS Dementia pathways (including Respite) 7 day working Joint Assessments ICT systems to support new working

VCSE Involvement Now and going Forward Now: – Catalyst representative on Health and Well-being Board – Catalyst representative on BCF Implementation Team – Healthwatch representative on Dementia Strand team Going Forward: – Greater involvement in the Multi-Disciplinary Service design / pathways design Rigour and accountability: – Regular performance and outcome information to inform future service design / demand / outcomes
