Phase I: Excel Tracking Tool+ (Option 1) Teams will begin by tracking their behavioral health & health measures in an Excel-Based Patient Registry. One Team Member will be responsible for managing the Excel Patient Registry. 10

Phase I: Excel Tracking Tool+ (Option 1) The Registry will have the same type of Dashboard metaphor for reviewing an individual case or group of cases. 11

Each Team will Receive… A copy of the Excel Patient Registry Training on the Registry User Manual Access to a Help Desk Ongoing Coaching Support Ongoing Population- Health Training 12

Phase I: Excel Tracking Tool+ (Option 2) Option 2: Each site would begin by tracking their behavioral health & health measures in their Electronic Health Record or Patient Registry. Using existing Dashboards built into the EHR/ Registry. 13

Phase II: dbMotion Rollout The power and capabilities of the dbMotion suite of tools will be rolled out There are modules for organizations with E.H.R.s (dbMotion Agent) and those still on paper records (dbMotion Collaborate) 14
