TIANA TAUILIILI COMMUNITY RESOURCE MANUAL

PURPOSE My purpose of this manual was to research in-depth FREE and LOW/REDUCED cost resources that fit the majority of indigent patients seen at the clinic.

GOAL To be used to increase the knowledge of the staff so they further help the patients’ needs that will help with their overall health and well-being.

OBJECTIVES I: The manual should be user friendly and easy to find what the needs of the patients are. The staff can talk with the patients about what needs these patients have and are able to discuss with them the options they have by having the knowledge of the resource manual.

SECTIONS Two different SUB-SECTIONS of FREE and COST/FEES: Shelters, Transportation, Medical Assistance, Food/Clothes, Mental Health/Counseling

WHAT’S ON EACH PAGE? Who-population served What-services provided When-seasonal service, only once a month, only every three months for a service…. How-phone number to contact

DIFFICULT? Always changing Supervisors, eligibility, fees, phone numbers, addresses, and other little details the make a difference Because it’s changing, the best is to call the organization/company and ask. CALL, CALL, CALL---never assume.

PATIENT SCENARIO #1 32-year old pt., alcoholic, male, unstable family, homeless sometimes, unemployed, lives in Athens area, gets to appointments by random friends that help

PATIENT SCENARIO #2 27-year old pt., female, 2 kids under 12, unemployed currently (can’t keep job because of diabetes), can’t afford diabetes medication, kids need clothes, food help living in different friends’ homes (has to constantly move because of space)
