2 What are the aims of the programme? www.local.gov.uk Outcome focused: ensuring people get the care they want & need Accountable: in partnership with & accountable to people that use services & their family carers Challenging: change culture as well as services Delivered at pace & scale: Fast & lasting change from childhood Sector-led: using existing structures & practice Open &transparent: ensuring that information on progress is frequent & accessible.

3 How will we do this? Two year funding for joint programme board Giving people & their families have a say Support both health & social care to change Work together – ADASS contribution Get people out of assessment & treatment centres Alive to challenges: resources, NHS change, scale of ambition, competing priorities

4 And when by? “Health and care commissioners will: by 1 June 2013, working together and with service providers, people who use services and families review the care of all people in learning disability or autism inpatient beds and agree a personal care plan for each individual, based on their and their families’ needs and agreed outcomes; put these plans into action as soon as possible, so that all individuals receive personalised care and support in appropriate community settings no later than 1 June 2014; ensure that all individuals have the information, advice and advocacy support they need to understand and have the opportunity to express their views. This support will include self-advocacy and independent advocacy where appropriate for the person and their family.” ADASS co-signatory of national concordat

6 Reviews of care Document on principles of reviews: Scope: Know who people are, what they are in, where they are placed – & what is needed to get them out of there Joint: reviews must be across health & social care Inclusive: need to ensure engagement & advocacy Comprehensive: good practice but regional variations

7 Understanding & changing care Impact for commissioners Analyse outcomes of reviews Support future planning Development of new services & approaches Share innovation Link with HWBBs to challenge & change
