Learning goal: to understand current approaches to treatment. To see how treatments are used provide holistic care. Success criteria: Participate in solving a case study by making a care plan; Help your client become “well”! APROACHES TO TREATMENT AND CARE PLANNING

Approaches to Treatment Conventional Medical Treatment Complementary/Alternative Treatments Mostly found in medical facilities. Focuses mostly on the diagnosis and treatment of disease or illness (broken leg, heart attack, Cancer). The treatments aim at reversing or repairing the underlying illness. If a cure is impossible, then the doctor/therapist tries to manage or ease the symptoms as much as possible. Offers some attention to prevention. (eg. Medication, stitches, treating a heart attack) Focus is on personal wellness, and improving wellbeing beyond treating disease alone. Recognizes belief that maintaining good health is a personal responsibility affected by lifestyle. Many alternative therapies, which were once dismissed as “hokum” are now considered complimentary treatments to conventional medicine. Alternative therapies are important for prevention and treatment of diseases. (eg. chiropractic, naturopathic remedies, acupuncture)

Why do you think it would be important to offer both approaches? Complementary and conventional treatments together offers a ‘holistic’ treatment approach. Holistic health care: Care that promotes physical, emotional, social, intellectual, and spiritual wellbeing by treating the WHOLE mind, body and spirit. *This is the new approach in health care! Together, these treatments offer a lot of choice. People can pick what works best for them. You have a better chance of achieving wellness.

Care Planning Here are the steps to caring for a client with health problems: 1. Take a history – What is the complaint? - What happened? - Find out background information – habits… 2. Do physical examination – look for clues to the problem: look, palpate (feel), listen, smell (use the senses) 3. Use diagnostic tests – Take samples from the body to analyze (blood, urine, feces, sputum or spit up, spinal fluid, tissues) - Diagnostic imaging (x-ray, MRI scan, CT scan) - Organ function tests 4. Make a diagnosis 5. Give treatments: conventional and alternative* (follow a written plan) *These may be offered at different locations depending on what is suggested. For example: yoga classes or massage therapy for stress management. 6. Monitor (take vital signs, retest body fluid samples) 7. Evaluate effect of treatment. Reconsider plan if not working.

KEY POINTS: This plan is ‘inquiry’ based to ensure a methodical, scientific approach to solving medical problems. Communication by use of a centralized system (patient chart - written notes and computerized) is essential so everyone knows the plan and can add information for others to consider. Effective care planning and documentation leads to high quality care. It takes a multi-disciplinary ‘team’ to provide all the care. Teamwork is essential!

Case Study CASE STUDY: Your friend is a victim of a drunk driver. Their car was hit head on by another car that crossed the center line. Their head hit the dash board, side window then snapped back against the seat. Their legs were pinned by the crumpled hood pushed inward. Their body was hit by flying objects in the car. Injuries: Smashed jaw and 2 missing teeth. Both legs are broken. Internal bleeding from the force of impact Cuts bleeding on your face arms, and legs from glass. Head injury with large bruises on the side of your head. (assume breathing) ADD NUMBERS TO THIS LIST TO PRIORITIZE. (MOST IMMEDIATE AND MOST SERIOUS)

Problem list: (use prioritization) 1 2 3 4 5 6 7

Example of complementary Tx: MUSIC THERAPY Story of Henry - - Alive Inside (6:00) https://www.youtube.com/watch?v=Fw7Y78aqf_I Now let’s explore the range of complementary treatments available and then apply them to this case.

Treatments Conventional Complementary/Alternative 1. Neck brace (for possible spinal injury) 2. Surgery (for internal bleeding) 3. Bandages, stitches (for bleeding) 4. Splints, casts, jaw wired, dental care, implants? (for bones) 5. Pain medication (“analgesics”)(for pain) 6. Physical therapy (for rehabilitation of mobility) 7. Psychologist, Social worker (for mental trauma)

1) Have we treated the mind, body, and spirit? 2) What is this kind of care called? 3) Why do you think it would be important to offer both kinds of treatments? 4) Can holistic care be provided by using all conventional or all complimentary treatments? 5) Can one person provide holistic care? 6) Next slide: Identify type of treatment approach: complementary or conventional. Key Concepts:
