5 Emerging model for prevention
Rationale High level of deprivation across the town and the variation in health inequalities between wards Growing austerity measures impacting on service provision, often leading to deterioration of poor health and the subsequent demand management issue   Significant work within public health around remodelling preventative services has taken place over recent months, including the decommissioning of some services Evidence suggests that individuals often present with multiple issues, therefore there is a greater need for integrated services

6 Health and Wellbeing Hub
Operated from a town centre location that has the opportunity to combine the following : integrated ‘one-stop-shop’ approach increased access and uptake of a wide range of public health preventative commissioned services, such as weight management and nutrition, physical activity, smoking cessation, mental and sexual health services; a social prescribing offer which will strengthen the skills and ability of the voluntary and community sector to respond to increased demand enhanced offer for clients with complex needs requiring a pathway of enablement support broader support services from housing providers, Job Centre Plus, Citizen’s Advice Bureau and voluntary/community groups all providing in-reach support and advice Timescale for completion and transition of services – July-September 2016
