A Prescription ( ℞ ) “a health-care program implemented by a physician or other medical practitioner in the form of instructions that govern the plan of care for an individual patient”

Prescriptions Prescriptions have legal implications, as they may indicate that the prescriber takes responsibility for the clinical care of the patient & in particular for monitoring efficacy & safety

Prescriptions However, as medications have increasingly become pre–packaged manufactured products & medical practice has become more complex, the scope of meaning of the term "prescription" has broadened to also include clinical assessments, lab tests, & imaging studies relevant to optimizing the safety or efficacy

Medical Prescription should Mention 1. Name & Dr address or hospital/clinics 2. Date 3. Clear 4. Concentration or amount: mg, mL, mg/mL, µg, UI, % g, mEq 5. Amount of dosage form in Rome letter 6. Dosage form & Volume of packaging: tube 10g, Fls 500 mL, tab 25mg, cap, vial, ampoule etc.)

Medical Prescription should Mention 5. Signature: using Latin language (Stdd tab I) 6. Administration (ac, pc, s.u.c, etc) 7. Official abbreviation: CTM, Na Bic etc. 10. Sign of doctor on each R. For narcotics drugs should be signed. High dose: should be signed behind dose required. 11. Patien’s name: Adult: should be wrote Tn, Ny, Nn; Pediatric: age; Geriatric: age. 12. Address: clear & complete.
