3 T IMELINES Annual Reviews Minimum of once every 365 days (one year minus one day) Policy 2419, Chapter 5, Section 3.A Reevaluations Within 3 years of the last EC report Policy 2419, Chapter 3, Section 2.B Additional Evaluations Within 60 days from parental consent until and EC or IEP Team must convene to review Policy 2419, Chapter 3, Section 2.C IF assessments are to determine eligibility – EC must convene IF assessments were for programming purposes or a related service – IEP Team must convene PWN At the conclusion of the IEP Team meeting, provide to parent Policy 2419, Chapter 5, Section 2.M

4 D ATA E NTRY Testing Accommodations Testing conditions and accommodations, as appropriate, are documented in the student’s IEP and WVEIS Special Education Student Information Record. Policy 2419, Chapter 5, Section 2.I Recommendation: Print WVP.326 report and verify WVEIS matches IEPs.

5 IEP I SSUES Inaccurate data on IEPs Recommendations use on-line IEP pulling data from student information in WVEIS verify basic student information at IEP Team meeting as IEP is being reviewed Periodically run reports and have teachers verify student information Weak Present Levels Recommendations refer to Chapter 5, Section 2.D for Present Levels content refer to IEP directions utilize the on-line IEP program and access the supports available such as the Mastery descriptors and CSOs Incorrect Goal Parts Recommendations view the Teach IEP site section describing goals refer to Policy 2419, Chapter 5, Section 2.E for Goals content

7 GED I SSUES When does change of placement occur? PWN must be provided when a student graduates with a standard diploma Policy 2419, Chapter 3, Section 2.B Recommendation: provide PWN if the student obtains a GED and will no longer be receiving special education services
