3. History 34 year old female with history of panic attack, asthma and depression with chief complaint of migraine headaches. Experienced migraines since the age of 17 and they have become more devastating the past three years. Under the care of two neurologists, the most recent being a headache specialist. LP, MRI, MRA all negative.

4. History Cont. Medications over the past three years include: Zanaflex, Wellbutrin, Lexapro, Relpax,, Neurontin, Zoloft, Seasonale, Zomig, Zofran, Ambien, Imitrex, Oxycodone, Hydrocodone, and Bioidentical Hormones. Quality of Life: Almost bed ridden, limited driving, depressed and afraid of pain. Previous treatments include: Acupuncture, Massage Therapy, Biofeedback, and Chiropractic Care.
