4 At present we support up to 112 service users. This ranges from supporting people in their own homes to supporting people who live in a group setting. The hours that we support people vary. We support one lady to live in her own home 24hrs a day and support another 2 hours a week. We aim to meet the individual needs of the service users. Some people require us to assist with the full aspects of daily living whilst other require minimal assistance to ensure that bills are paid. All service users have their own individual personal centred plan.

6 What Do We Offer As A Service? Above all we offer personalised packages of support to you based on your needs and aspirations This means that you choose what service you want to receive from us. Our aim is to work with you as an individual Respecting your right to make your own choices both about how you live your life and the services you receive from us Always working with you to maximize your independence Supporting you to achieve your goals and aspirations

7 To achieve this we will: -Listen to you -Work in partnership with you and others you want involved in your circle of support -Offer practical support and enable you to develop your own confidence and skills to make the most of the opportunities available to you -Be flexible and adapt to your changing needs and aspirations

8 Individual Support Plans Everyone we support has a Person-centered Plan. This is your support plan, developed by you with the support of your worker and with the involvement of any others you want to be involved e.g. family and friends. It will cover the outcomes you want form the service, which may include help with personal care and maximizing what you can do for yourself; managing your health issues; making new friends; accessing facilities in your community; gaining education; accessing volunteering and employment and moving to or staying in your own home.

9 Your person-centered plan: -Focuses on enabling you to reach your goals and aspirations -Helps you to understand and manage the risks that may be involved in the choices you make -Sets out what practical support, emotional support and advice you may need from us and from others -Is reviewed regularly, whenever you wish and as your needs and aspirations change -Enables you to see the progress you are making towards the goals you have set for yourself

10 Hello. My Name is Pat 1. The staff that come they sometimes make a menu list with me to follow. 2. The staff help me to write my next weeks shopping list for when we go shopping. 3. We make a cleaning rota for me to do my flat. 4.I ask them to sign my tablet board when I have taken my tablets. 5. Prospect have supported me for approximately 12 years. I can manage on my own but need the support that Prospect give me to make sure that I am keeping on top of things. I also have a voluntary job helping at the YMCA which I really enjoy.
