3 Eligibility for Benefits All full-time employees (30 or more hours per week) are eligible. Must meet 90-day employment. Effective 1 st of the month following 90 th day. Part-time employees and contractors not eligible.

5 Basics of the benefits offered Health Insurance –Base Plan – Free for employee only coverage High deductible Coverage for children and/or spouse –Buy Up Plan – $42 for EE only, $70 EE & Child, $118 Family** Low deductible, co-pays for doctors and medicine Coverage for children and/or spouse **Premiums are on a weekly basis

8 Basics of the benefits offered Life All employees receive $20,000 life insurance Additional life available up-to $500,000 Coverage available spouse and children at 50% of employee level AD&D (accidental death & dismemberment) Coverage available $10,000 to $100,00 Coverage available for spouse and children at 50% of employee level

10 Basics of the benefits offered Short-term disability Payments begin after 7 days off Coverage for up-to six months Premium based on hourly wages Long-term disability Coverage begins after six-months of disability Premium based on hourly wages

11 Basics of the benefits offered Accident Payments for injuries not related to work Cancer Payments for diagnosis and treatment of cancer Limitations based on type of cancer Critical Care Payments treatment of heart attack, cancer, stroke, and severe burn injuries **Coverage is available for EE only and options are available for spouse and/or child(ren)

12 Deduction Questions When will deductions begin? –Deductions begin the first week-ending date in the month effective. Example: Effective 11-1-12, Deductions start week-ending 11-4-12. –Note – coverage is effective 1 st of the month following 90-days What happens if an employee misses work? –FMLA and Workers’ Compensation absences are protected. Benefits are required to be offered in the same manner as if the employee worked. –LOA or other absences Employees are responsible for full premium after one week of absences.

13 Summary Eleven different insurance benefits available for all full-time employees. There are base and buy-up options for Medical and Dental. $20,000 life insurance for all full-time employees. Eligible after 90 days. Effective on the 1 st of the month following 90-days Certain absences are protected.

15 Assessments The matching game results will go into the individual employee’s file. The summative assessment (benefits matching game) will be used to determine that the administrative staff understand the basics of the benefits offered. The assessment will be graded by the benefits administrator and/or the consulting group that offers the benefits. Should an employee need further clarification, they will meet with the consulting group that offers the benefits.
