3 Introduction UTMB  the state’s first academic medical center, established in 1891  four health sciences schools  three institutes for advanced study  a research enterprise that includes one of only two national laboratories  a health system offering a full range of primary and specialized medical services  a component of the University of Texas System and a member of the Texas Medical Center The Center for Obesity and Metabolic Surgery COAMS of UTMB currently offers a non-surgical program for patients who are either not eligible for, or do not wish to pursue, bariatric surgery. To fully support patients on a very low calorie diet (VLCD) and ensure the consistency in adherence to the prescribed diet plans, COAMS would like to offer patients medical nutritional supplements in the form of meal replacement bars, shakes, snacks, or food that supports a VLCD of 800 calories per day. 3

5 Scope of Work / Service Requirements 4.2 Scope of Work UTMB desires to solicit manufacturer proposals for wholesale pricing of all medical nutritional supplements and/or meal replacement products that support a very low calorie diet (VLCD) of 800 calories per day. This includes, but is not limited to, meal replacement bars, shakes, smoothies, drinks, beverages, snacks, desserts and other products appropriate for patients on a VLCD. Minimum Requirements (Section 4.2.1) 1. Supplier should be reputable, meet national safety and quality standards. 2. Supplier should offer several types of both solid and liquid meal replacement foods: At least 3 different flavors for the liquid meal replacement or shake (example: vanilla, chocolate, and strawberry) At least one savory liquid meal replacement option (example: chicken soup) At least one clear liquid fruit drink At least 5 flavors of solid meal replacement bars Each meal replacement bar or shake should contain no greater than 160 calories each, and no less than 15 grams of protein each. They should contain less than 3 grams of lactose per serving. 3. All products should be shelf-stable, requiring no refrigeration. The shelf life should last no less than 12 months. 4. Respondents shall confirm they are willing to provide samples of products listed above for evaluation at UTMB’s request. 5

7 Historically Underutilized Businesses (HUBs) (Section 1.14) UTMB has reviewed this RFP in accordance with the Texas Administrative Code Section (Title 34, Part 1, Chapter 20, Subchapter B, Rule 20.14) and has determined that subcontracting opportunities are not probable under this solicitation. If your company does intend to subcontract any portion of the work, your proposal must include a “HUB Subcontracting Plan” (HSP). Respondent’s exceptions will be reviewed by UTMB and may result in disqualification of Respondent’s proposal as non-responsive to this RFP. General exceptions are not allowed. Please explain the reason of the exceptions. 10
