2 Goals of our monthly webinars Our goals for our MFP monthly webinars are:  To provide training on key topics  To better the lines of communication between you and us, the MFP staff  To seek feedback on how to better the processes of MFP/waiver services for our participants  To provide the opportunity to share successes and challenges as RTC’s and MFP transitions

3 MFP Transition Numbers As of today September 10, 2015  152 referrals have been received  46 participants have transitioned to date (15 in 2014 and 31 thus far in 2015) * Our benchmark for 2015 is 47 transitions. * We have met or have exceeded our bench- marks in all waivers except DD and PRTF.

4 MFP and Reimbursement for Transition Services Waiver Transition Services vs. MFP transition services: How are they the same how are they different?  MFP allows for security deposit and 1 st month rent.  MFP has a more generous fee structure  MFP has been more lenient in purchasing household goods  MFP transition services are time limited to 30 days post move MFP allows agency pre-payment of cost of household goods. MFP has developed policy to provide to pay for security deposits and first month rent if needed.

5 Re-institutionalization If an MFP participant is re-institutionalized during their 365-day period of MFP eligibility they may remain enrolled in the program and resume their participation upon discharge. These participants are entitled to receive their 365 days of services under MFP, even if not consecutive because of re- institutionalization. Participants readmitted to an institution for 30 days or less remain enrolled in Montana’s MFP program. MFP participants remaining in an institution for more than 30 days will be suspended from Montana’s MFP program. The State will reactivate these suspended participants if they transition back out of the institution into the community. These participants do not need to meet the 90-day residency requirement to be reactivated. At this point, individuals upon discharge from inpatient care resume their status as an MFP participant and are eligible to receive MFP services for any remaining days up to the 365-day demonstration participation period. No inpatient days are included in the 365-day MFP period of participation.

6 Successful Transition Story  Liz Davies with Community Medical in Missoula served as the RTC.  Participant transitioned from a Missoula Health and Rehab nursing home to a subsidized apartment in Missoula in April 2015.

7 Other Items RTC Linda Simmons shares her medication snafu when transitioning her participant from a GF NH to a Helena apartment? Exiting MFP:  When your MFP participant concludes their 365 days of service, MFP Transition Coordinator will send a letter out to the participant and/or legal guardian and cc the RTC 1 month prior to their end date informing them of the completion of grant funding. Contact information on moving/cleaning companies: Right Now Cleaning – Butte – Wanda Matter – 406.565.2034 – Fax: 1.866.492.5816 Contractor – Medicaid enrolled provider – statewide service – Nichols Design and Build Owner – Rodney Nichols – 406.560.8834 - (Butte) Bureau of Indian Affairs has a Housing Assistance Program  Apply for assistance in December or January and assistance is approved in July of that year. It is a simple and short application process. If approved the participant can receive up to $35, 000 in repairs to their home. The BIA sets the priority on repairs.
