4 Care Status (Problem List) CARE PLAN at initial Medical Home visit for baby and family 1. Pass hearing screening 2.No known risk factors 1.Conduct risk factor assessment 2.Discuss milestones for speech and language 3.Assess middle-ear status, with pneumatic otoscopy or tympanometry at well-child visits 1. Conduct additional screening if there is parental concern for speech and language development at any well- child visit 1.Pass hearing screening 2.Known risk factors 1.Conduct risk factor assessment 2.Discuss milestones for speech language 3.Assess middle-ear status with pneumatic otoscopy or tympanometry at well- child visits 1. Conduct additional screening if there is parental concern for speech and language development or with special conditions* (AAP and JCIH) 1.Refer for audiologic diagnostic assessment between 24-30 months of age

Hearing screening not complete (includes technical fail ) 1.Conduct risk factor assessment 2. Discuss developmental milestones for speech language 3.Assess middle-ear status, using pneumatic otoscopy or tympanometry at well-child visits 1. Referral to the birthing facility or another outpatient screening facility to complete the screening before one month of age
