3 KO AU - Adapting and improving your individual practice  Able to differentiate between transacting and transforming  Clear and accurate documentation noting a baseline with clear identification of needs and goals  Self analysis and evaluation of work to ensure it evidences whānau transformation  Seeking supervision/professional support to ensure that practice is of the highest standard

4 KO TĀUA - reviewing individual working relationships ensuring they are supportive of reaching whānau outcomes  Acknowledges the importance of relationships at an individual level  Ensures kaimahi are working together to meet the needs of whānau  Actively build relationships with colleagues to enhance the whānau experience and outcomes  Working together to eliminate barriers and find solutions  Focus on what can happen rather than what can’t happen

5 KO MĀTAU - adapting the organisations approach streamlining systems making the whānau journey as smooth as possible  The need for the organisation or collective to build a systems approach  Single and shared data entry points reducing duplication  Reviewing practice and process to reflect a single connected journey e.g single referral process, single data entry point, streamlined documentation  Developing shared case management where required to improved outcomes for whānau  Inclusive and empowering whānau

6 KO TĀTOU - working collectively as communities developing a systems approach to ensure whānau outcomes are evident within the population  Acknowledges the importance of relationships at organisational level  Sharing resources, education and training opportunities  Using a hub and spoke model to support workforce development and capacity building  Secondments and cooperative decision making  Strong leadership reinforcing the whānau ora approach across communities.
