1 Communicating the value of the work and the role of caregiver is essential. A caring team works together to promote harmony and healing among themselves as well as those they serve, influencing the whole system. Effective communication and collaboration between nurses, physicians and ancillary staff is essential to positive patient outcomes. This project focuses on addressing multidisciplinary collaboration and effectiveness in order to plan more efficient and satisfying discharge outcomes with patients. We intend to create a caring relationship with our patients that promotes knowledge, growth, and empowerment and enhances their awareness of health and well-being. This project is a part of our organization’s effort in bringing human caring science into the active practice of nursing staff in a 40 bed Medical-Surgical/Telemetry- Stroke unit. This project was designed to improve the way we round on our patients, discharge process and patient flow (ED-Bed). The nurse manager and assistant nurse managers utilized a valuable approach in rounding by creating a rounding tool that emphasizes the caring-healing relationship between the nurses and the patient. I would like to acknowledge Kaiser Permanente’s leaders for bringing the Caring Theory to our medical center. Deep appreciation to ninth floor staff and management team in embracing the positive changes in the unit. The purpose of this project is to develop helping, trusting and caring relationships with patients, families and the members of the health care team. Our aim is to improve the patient flow and service experience of our patients through effective communication and creation of a trusting relationship among caregivers. We developed a tool to measure the caring/healing relationship between the health care team and the patient. This tool includes assessment of the patient’s perceptions of caring, nurses caring behaviors, abilities and efficacy. It also helps to transform tasks into caring healing interactions, creating a higher level of consciousness. Healing and wellness are facilitated through an efficient discharge process. Another graphic or chart can go here The Care rounds statements were focused on : Communicating the daily routine and expectations on newly admitted patients. Asking if they know who is caring for them that day and if the nurses are responsive to their needs in a courteous and respectful way. Asking if they are kept informed about their daily care and pain management. Patients were asked about the consistency of the health care team in providing care with loving kindness and patients were asked if they felt that their cultural traditions and spiritual beliefs were supported. Through consistent care rounds we assess the patients needs, experience, and gauge R eadiness for Discharge : Patient’s discharge plan is discussed as early as admission. We review the next day targeted discharge list, track and monitor the progress of discharge readiness. We follow-up on action items to facilitate next day timely discharge e.g. pending tests, teaching, PT eval, transportation arrangement s and DME (Durable medical equipment). In AM we project the patients who potentially will be discharged in time groupings ( 1500H). Developing a Helping-Trusting Caring Relationship Vanessa A. Decreasing and eliminating overcrowding and surge alerts of admissions during late afternoons. Receiving positive feedback from our patients for the excellent caring experience they had during their hospital stay: quotes like “I want to take my Nurse home with me” Receiving nurse award recognitions from patients two months in a row for going above and beyond with care. Linking staff engagement with all our initiatives in quality, safety and patient satisfaction through skin and fall rounds, no HAPU (hospital acquired pressure ulcer) and CLBSI (central line blood stream infection). Increasing patient satisfaction service scores to six points. Decreasing patient overall length of stay
