2 What is the Partnership for Patients (PfP)? Created by the Affordable Care Act, the Partnership for Patients (PfP) is a nationwide public-private partnership to improve the quality, safety and affordability of health care for all Americans PfP brings together leaders of major hospitals, employers, physicians, nurses, and patient advocates along with state and federal governments PfP now consists of more than 6,500 partners, including: over 3,167 hospitals; 2,345 physician nursing and pharmacy organizations; 892 consumers and consumer groups, and 256 employers and unions

3 Partnership for Patients: Better Care, Lower Costs Nationwide public-private partnership to tackle all forms of harm to patients. Our goals: 40% Reduction in Preventable Hospital Acquired Conditions over three years 1.8 Million Fewer Injuries 60,000 Lives Saves 20% Reduction in 30-Day Readmissions in Three Years 1.6 Million Patients Recover Without Readmission Bonus: Potential to Save $35 Billion in Three Years

6 PATIENT PERSPECTIVE Ensure the patient perspective and voice is part of every PfP effort, and patient and family engagement (PFE) is integral to reaching the PfP aims Identify emerging best practices, tools, resources and innovations in PFE and developing effective mechanisms to disseminate and create action Encourage rapid adoption of best practices and innovations in PFE PFE Areas of Focus

7 PFE Network Goals Establish a core group of patient advocates to ensure the patient perspective and voice is part of every PfP activity Create a growing network of advocates who can partner with HENs, especially at the community level, to advance PfP goals Collaborate with other partners to support broader patient education, outreach and action to advance PfP goals

9 What We’re Learning There are a wide range of ideas about emerging best practices and innovations, and the ideas cluster into a model with three general categories: –Point of Care –Policy & Protocol –Governance Most groups are doing patient outreach and report using a wide variety of tools to communicate about patient safety –Hospitals report that their patient and family engagement efforts have helped them achieve measurable positive trends in reducing medical errors and hospital readmissions
