2 Overview History of SCT, NHS 24 and SCTT NHS 24 current Mental Health Services Dementia workstream

3 SCTT SCT formed in 2006 by SGHD £1M recurring funding SCT merged with NHS24 1st April 2010 National Telecare Development Programme (TDP part of JIT) was transferred into NHS 24 and merged with SCT from 1st April 2011 Now known as the Scottish Centre for Telehealth & Telecare

4 SCTT 4 National key programmes of work: Stroke & CVD LTCs with a focus on COPD Mental Health Paediatrics – funded separately via NDP

5 SCTT Supported by National infrastructure plan Training & Education Convergence between health & social care - Telehealth & Telecare - SCTT

6 Mental Health Strategic Framework Based on NHS 24’s workstreams -Providing unscheduled care -Improving Health -Improving access

7 Shetland Dementia Service 1 WTE specialist nurse/Dementia Services Development Manager 1 session Consultant Old Age Psychiatrist ‘Dementia-only’ service Initial assessment, diagnosis, advice on management & follow-up Use of technology in providing supervision, direct clinical care and education Future expansion to include other conditions that fall under the remit of Old Age Psychiatry Assisted NHS Shetland to meet HEAT Target for Dementia

8 Shetland Dementia Service No clear pathway for diagnosis and management Managed by GPs, hospital doctors and social workers with little specialist input If specialist input was necessary, patients would have to be transferred to Aberdeen

11 Dementia Services Feasibility Study – proof of concept Aim - To provide specialist assessment and treatment for care home residents with mental health problems and to offer advice, support and education to care home staff utilising video conferencing facilities.

13 Dementia Services - Aims To improve management of behavioural and psychiatric symptoms of dementia (BPSD). Improve recognition and treatment of functional disorders in the care home population. Improve end of life care/palliative for elderly patients with mental health problems.

14 Dementia Services – Future Developments Cognitive stimulation therapy via VC Falls prevention work Ongoing CPD for staff Links with Iris Murdoch Centre
