3 Why do we do STAC’s? 4 EXPECTATIONS * Everyone has a firm understanding and is able to train/coach others on the STAC process. * Everyone has an understanding of accountability within the STAC process. * Everyone is using task analysis on every job. * Everyone is using task analysis to achieve the desired behavior “ Zero Incidents”. * Everyone has the ability to recognize hazards, elimination's and controls.

5 Contractor foreman/superintendent will be held accountable for ensuring their employee’s receive training on STAC and for the quality of the cards. Deficiency letters will be issued to your company for the following things: STAC score of 2 or less Task being performed not listed on STAC No/Wrong date on the STAC Foreman/ Crewleader has not signed off on the STAC No STAC EXPECTATIONS

9 Section 1 - Task Analysis Why do we do task analysis? Task analysis is conducted to look at the job in small parts, to identify hazards, and eliminate and control the hazards. Task Analysis Analyze the job tasks - 3 to 5 * Break job down to at least three tasks to be sure you are not analyzing too much of the job at a time. * There should be no more than five tasks, this is not a job procedure. * Group similar tasks to eliminate repeat of hazards: example - Loosen lugs/tighten lugs Remove/replace pump

10 Section 2 - Identifying Hazards Hazards Must Be Specific To The Task: - back strain from lifting 6” pipe - cuts from burrs on pipe - tripping over air hoses in work area - burns from hot welds MAKE SURE HAZARDS ARE DIRECTED TO YOU OR OTHERS

11 Section 3 - Elimination & Control Section 3 - Elimination & Control Elimination is the removal of the hazard from the work area Control is the reduction of the risk associated with the hazard (PPE, etc.) You must identify an elimination and a control for every specific hazard that was identified. You must be specific in the controls that are identified. Note: PPE is always a control. * leather gloves * ear plugs * monogoggles * rubber boots Have to ask the question of every hazard: * How can I eliminate the hazard? And just to make sure - if the hazard is not eliminated: * How am I going to control the hazard so that it does not hurt me?

12 Why have a STAC scoring system? To measure strengths and progress in task analysis - individual and company To identify weak points and address with focused training/coaching

13 STAC’s that are filled out in Spanish will have to be explained to the observers, in English, by the individual who owns the card. **** NOTICE ****
