2 LMI Case Management Goals The primary goal for case management is to link individuals and families to services and supports that impact their stability and sustainability through access to safe and affordable housing, community partner agencies, mainstream benefits and cash assistance The sustainability of individual households should have an overall impact on the stability of the greater community.

4 Lakewood Community Services Center LMI Case Management Of the total number of clients who have presented with the need for case management and referrals this year, 34% are single female heads of household 17% (up from 11% in 2014) are adults age 62 and older These vulnerable segments of the population need multiple services over a long engagement period CDBG funding is for low-moderate income residents; YTD July 2015, a full 85% of the clients receiving case management fall into HUD’s extremely low income bracket These are extremely at-risk households who need multiple services Individuals and families who are new to Lakewood and/or new to the benefits system, need ongoing assistance to access services available in our community and beyond

6 Lakewood Community Services Center LMI Case Management What makes LCSC uniquely positioned? Multiple support services are available on site at the Center: - Murtis Taylor staff is onsite daily for mental/behavioral health assistance - Towards Employment is onsite daily in the Networks Job Resource Center - CEOGC has outreach worker onsite year round for access to utility assistance programs - CTO onsite twice a month to assist in the resolution of landlord/tenant issues - CHLAP attorney available once a month for free legal assistance with civil issues Clients come frequently for food, making barriers assessment and follow up relatively easy for improved client outcomes

7 Lakewood Community Services Center LCSC Uniquely Positioned LCSC is unique in that multiple support services are available right on site at the Center. Murtis Taylor staff is onsite daily for mental/behavioral health assistance; Towards Employment is onsite daily in the Networks Job Resource Center; an outreach worker from the Council for Economic Opportunity in Greater Cleveland (CEOGC) has an outreach worker onsite year round for access to utility assistance programs; Cleveland Tenants Organization (CTO) is onsite twice a month to assist in the resolution of landlord/tenant issues; and an attorney is available once a month for free legal assistance with civil issues. Because clients often come in for food every month, LCSC social workers have a unique opportunity to follow up with them to measure their success in accessing support and to assist them become more successful.
