4. Submission Send one copy electronically to your AIDD Project Officer via email and one original hard copy to your Grants Specialist You can find the Project Officer and Grant Management assignment list on URC website

5. Final Progress Report Section 1: Introduction Section 2: Organizational Profile Section 3: Evaluation Methodology Section 4: Final Report on UCEDD Accomplishments Section 5: Funds Leveraged Overall Section 6: Summary of Collaborative Efforts with the DD Network Appendix

6. Process Project Officer will review reports October through December Project Officer will contact ED with questions and feedback Grants Management Specialist will review financial reports and close out grants after receiving approval certification from Project Officer
