2 Trainer Requirements Integrity checklist Commitment to training Trainee competency checklist Final evaluation of training Practice test Incident reports Finishing evaluation report BTW Log Macro 54 Weekly check calls Coordinator notification As we wrap up the training class, we need to discuss some requirements of you while in training. You will have some documentation that you will need to complete, and some that you will need to ensure the student completes. There will be required calls to the training department that will need to be made by you and/or the student while training.

3 Paperwork There will be training paperwork that you and the student will need to complete during training. Acknowledgement and Consent Form Integrity Checklists. Student/Trainer Evaluations. Commitment to Training. Competency Checklists. Program Evaluation. BTW Logs. Incident Reports. Every student should receive a student manual upon completion of orientation, and the forms that we are going to address are in the manual. It is a good idea to make extra copies of the forms just in case you pick up a student who did not receive a manual. As you can see there are a number of documents that you and your student will need to complete during the training cycle.

4 Paperwork Completed by you and the student - letting us know if you want to be paired with members of the opposite gender. The acknowledgement and consent form is where you and the student will let us know if you want to be paired with members of the opposite gender during the training cycle. Please complete the form and turn it into the instructor.

5 Paperwork Read each statement of the trainer integrity checklist then enter your initials in the space provided, indicating you understand, agree, and will comply with the program guidelines. The Trainer Integrity Checklist is you acceptance and agreement to comply with the guidelines of the training program. Please remove this document from your trainer manual, and read each statement. After you have read each statement, enter your initials in the block at the end of the statement indicating that you understand, agree, and will comply with the program guidelines.

6 Paperwork Commitment to Training
Review each statement with the student You and the student initial after the statement You and the student sign & date at the bottom of the form Give the form to the upgrade specialist at the time of upgrade The next document is the commitment to training. You and your student will complete this form, when the student first comes on the truck. This is a commitment to the program guidelines, and training objectives between you and your student. You need to go over each statement with the student, then both of you will initial the block after the statement. You will then both sign and date the bottom of the form. You will then put this form in an envelope that will be maintained until the end of the training period, at which time it will be given to the upgrade specialist.

7 Paperwork Competency checklist is your evaluation of the student’s progress. Six Categories: Pre-trip, Driving, Backing, Paperwork, Special situations, and Technology. Enter a start date when you start training, and a completion date when the student reaches proficiency. The trainee competency checklist is your evaluation of the student’s progress through the training cycle. The checklist is broken down into six categories: Pre-trip, Driving Position, Backing Portion, Paperwork, Special Situations, and Technology. You will start by entering a date when you begin teaching each of the tasks under the five categories. You will enter a completion date when you feel the student has gained the necessary proficiency in the task. You and the student will also enter your initials after the completion of each task in the space provided. Once all tasks have been completed to the necessary proficiency level, you and the student will sign and date the bottom of the checklist. You will then put this form in an envelope that will be maintained until the end of the training period, at which time it will be given to the upgrade specialist. After the student reaches proficiency on all tasks, you and the student will sign & date the bottom of the form. Give the form to the upgrade specialist at the time of upgrade

8 Paperwork Final Evaluation completed by the student, and turned into upgrade specialist at time of upgrade. Ensure student completes from, but do not review or help student complete the form. Evaluation is a tool used to strengthen the driver finishing program. The final evaluation of the training program is completed by the student, and turned into the upgrade specialist at the completion of training. Your responsibility is just to ensure that the student knows to complete the form. You are not review or help the student with the completion of this form. The evaluation is a tool used to strengthen the driver finishing program, wherein we are able to get honest feedback from the student on the positive and negative aspects of the overall training process from the students perspective.

9 Paperwork Incident reports: Completed on an as needed basis.
Training coordinators will often request an incident report when documentation of an incident is needed. The incident report is completed on an as needed basis in the event an incident occurs, on the truck, and documentation of the incident is needed. 10 Paperwork Finishing Trainee Evaluation
Your final evaluation of your student Student evaluated on 27-tasks, on a scale of 1-10 Must receive a score of 7-10 on all tasks to upgrade Finishing Trainee Evaluation The Finishing Trainee Evaluation is your final evaluation of the student. You will be evaluating the student from 1-10 on twenty-seven tasks that the student must be proficient on before upgrade. Students must receive a score of 7-10 on each task in order to upgrade.

Completed by the student and turned into the upgrade specialist at upgrade. Do not assist, or review the evaluation. The Finishing Trainer Evaluation is the student’s evaluation of you as a trainer over the course of the training period. The student will complete this and turn it into the upgrade specialist at the time of upgrade. You are not to review or assist the student in completion of this document.

12 Paperwork BTW log is student’s behind the wheel driving log. Maintained by student and must match correspond with his record of duty status. Validation of students hours for upgrade. Reviewed by upgrade specialist at time of upgrade. The BTW is the students behind the wheel log. It is the validation of the student’s driving hours for upgrade. The BTW log must be maintained by the student, just as the logbook, and MUST match the record of duty status or Elogs. You must ensure the student records their hours in the BTW log after each change of duty status. The BTW log will be reviewed by the upgrade specialist at the time of upgrade to ensure that the student has the correct number of hours for upgrade, and that it corresponds with both his and your record of duty status.

13 Written Exam As we previously stated the student will be required to successfully pass a written test at the time of upgrade. The student’s manual contains a study test that should help the student prepare for the exam. Please help the student to prepare for the test by going over the sample test questions with the student until they are proficient in answering the questions. Students will be required to pass a written test to upgrade. Working with the student by going over the test questions, until they are proficient in answering the questions, will mean success on the test.

In the “Upgrade Student HRS Field” enter a Y in either the super solo or team fields AFTER the student completes 85 BTW hours if you want to run super solo or team miles. DO NOT enter a Y in both the super solo and team fields. N It is very important that every day you submit a macro 54. The macro 54 helps the training department track the student’s hours, but also identifies when the first 75 hours have been completed, and you are eligible to run super solo or team freight. In the “BTW STUDENT HRS” FIELD, you will enter ALL the hours the student has driven since being on the truck or since he/she has been in the program. This is NOT how many hours the student has driven that day…. DO NOT ENTER A DECIMAL in this space, round down the student’s hours. Example; if the student has driven 45.8 hours, you would enter 45 into the BTW field. In the field “UPGRADE TO SUPER SOLO (Y/N)” or the “TEAM” field … These fields should always be “N” if the BTW hours is less than 85. Once you have 85 BTW hours you can then choose EITHER “Y” for Super Solo, or a “Y” for Team. Do NOT enter a “Y” in both fields, if you do this it will confuse the system and keep you from getting the miles desired when running super solo or team. C-1 students are required to complete 105 BTW hours before being able to run super-solo or team. Y TX Y
